SALARY RANGE $26.69 - $40.88WORK ASSIGNMENT DETAILS Waukesha County's Department of Health and Human Services is seeking a Children's Long-Term Support (CLTS) Service Coordinator to serve in a specialized capacity as a Provider Relations Specialist. This position builds upon the foundational knowledge and skills of a CLTS Support and Service Coordinator and plays a key role in strengthening our provider network. The CLTS Provider Relations Specialist serves as the primary conduit for communication and collaboration between Medicaid-authorized CLTS service providers and the Department. This specialized role supports the broader goals of the CLTS program by enhancing provider capacity, strengthening system partnerships, and ensuring high-quality, compliant services for children and families. CLASSIFICATION SPECIFICATION The department may choose to fill this position as a Senior Care Manager. Senior Care Manager Minimum Training & Experience Requirements Care Manager Minimum Training & Experience Requirements: 1) A bachelor's degree from a recognized college or university in sociology, social work, psychology, nursing, criminal justice, or a closely related field. 2) Valid driver's license. 3) One year of related work experience in social work, counseling, nursing, criminal justice, or a closely related field.
