Description Part Time APP UNC Lenoir's Transitions of Care Clinic UNCHMG is looking for a PT APP to join the growing UNC Lenoir's Transitions of Care Clinic as a virtual APP. Responsibilities: *Provide remote patient management and care, collaborating with healthcare providers. *Utilize electronic medical records (EMR) systems to facilitate communication and care delivery. *Manage patient communication, including scheduling appointments and sending reminders. *Facilitate video consultations and ensure timely responses to patient inquiries. Schedule: Two days a week Hours: 8am-4pm EMR: EPIC Qualifications: *Graduation from an accredited NP or PA school *One or more years of full panel primary care experienced required *North Carolina licensed *BLS & ACLS certified I look forward to assisting you with starting your career at UNC Health!
