Part Time APP UNC Lenoir's Transitions of Care Clinic

UNCHMG is looking for a PT APP to join the growing UNC Lenoir's Transitions of Care Clinic as a virtual APP.

Responsibilities:

*Provide remote patient management and care, collaborating with healthcare providers.

*Utilize electronic medical records (EMR) systems to facilitate communication and care delivery.

*Manage patient communication, including scheduling appointments and sending reminders.

*Facilitate video consultations and ensure timely responses to patient inquiries.

Schedule: Two days a week Hours: 8am-4pm

EMR: EPIC

Qualifications:

*Graduation from an accredited NP or PA school

*One or more years of full panel primary care experienced required

*North Carolina licensed

*BLS & ACLS certified

Education Requirements:
* Completion of an accredited graduate-level Advanced Practice Provider (APRN or PA) program.
Licensure/Certification Requirements:
* Licensed as an Advanced Practice Provider (i.e., Nurse Practitioner, Physician Assistant) in the state of North Carolina.
* Board certification if required by the Department. DEA.
Professional Experience Requirements:
* No prior experience required.
Knowledge/Skills/and Abilities Requirements:
* Demonstrated clinical competency.
* Effective leadership skills. Strong interpersonal skills.
* Ability to work independently and collaboratively.
* Demonstrated written and verbal communication and strong analytical skills.
* Effective interaction to ensure the successful care of patients in the clinical setting.
* Documentation in the medical record to substantiate accurate billing.
