What You Will Do

The Advanced Practice Provider (APP) will be an integral member of the multidisciplinary team at the Hemostasis and Thrombosis Center, providing specialized care to our adult patient population with congenital bleeding disorders and acquired factor deficiencies. Working in close collaboration with the adult hematologist, the APP will deliver comprehensive, evidence-based clinical care in both outpatient and consultative inpatient settings. This role includes: direct patient care assessment and management, order placement, surgical planning; participation in comprehensive care visits, including outreach and offsite clinics; includes the follow up of labs and plan; Rounding and/or consulting with inpatient team when HTC patient is hospitalized; providing education/training to professionals and consumers, including presentations and development of written materials. Participation in HTC community events and professional training programs is required. Participation and compliance in research activities (HTC and industry) is required.

Key Responsibilities:

  • Perform comprehensive assessments of adult patients with bleeding disorders, including history-taking, physical exams, and interpretation of relevant diagnostic results.
  • Develop and implement individualized care plans in collaboration with the adult hematologist and multidisciplinary team.
  • Manage treatment regimens, including factor replacement therapy, rebalancing agents, prophylactic protocols and gene therapy.
  • Coordinate and participate in multidisciplinary comprehensive care visits.
  • Lead and assist in surgical consult planning, including preoperative assessments and perioperative/ post operative/discharge & home therapy management in collaboration with surgical teams and home care agencies.
  • Place and manage orders, including for labs, imaging, procedures, and medications under the collaborative agreement.
  • Be available as first point of contact for questions about orders, scheduling, and lab work from infusion RN's, pharmacies, and administrative staff, with the ability to place and update orders in EPIC as requested in a timely manner.
  • Manage team inbox in EPIC for refills, patient advice requests, abnormal lab values, signing of pending orders, assistance with prior authorization and peer-to-peer requests for medication approvals.
  • Provide patient and caregiver education regarding disease processes, treatment regimens, and preventive care.
  • Work collaboratively with physicians, nurses, social workers, pharmacists, genetic counselors and physical therapists to ensure seamless patient care.
  • Contribute to HTC quality improvement initiatives and clinical research activities as appropriate.
  • Maintain accurate and timely documentation in the electronic medical record (EPIC).
  • Engage in continuing education to remain compliant with credentials and current with clinical advances in hematology and bleeding disorders.

Working Conditions:

  • Outpatient clinic-based with inpatient consultation as needed. Outreach clinics involved and offsite clinic possibility.
  • Standard daytime hours for clinics, with flexibility to accommodate procedural planning or urgent needs.
  • Participation in some community educational and outreach activities related to the HTC is required.
