This position involves providing skilled nursing care at Metro New York DDSOO community sites in Manhattan and Bronx counties, particularly at medically-enhanced locations.


Key Responsibilities

  • Deliver professional nursing services to patients.
  • Assess, plan, and implement client care following the Plan of Care or Doctor's orders, which may include:

    • Dressing changes
    • Insertion and irrigation of urinary catheters
    • Oxygen administration by mask or cannula
    • Tracheostomy care and endotracheal suctioning
    • Gastrostomy tube feedings
    • Gastrostomy/Jejunum tube medication and flushes
    • SQ or IM injections
    • Insulin administration
    • Obtain specimens
    • Nephrostomy tube flushes
    • Colostomy care and/or irrigation
    • Administer CPR and other emergency procedures as necessary
    • Provide staff training and in-services


  • May assume the role of staff development nurse or perform episodic nurse duties when needed.
  • Step in to perform LPN duties as required.
  • Supervise all LPNs clinically.
  • Engage in private duty nursing or visiting nursing for short-term specialized services, such as:

    • Diabetes care (insulin injection)
    • Wound care (complex wounds/wound VAC/pressure ulcers)
    • Tube feedings
    • Trach care


  • Complete all nursing tasks comprehensively and timely, including:

    • Efficiently completing assignments within allotted time frames
    • Providing backup nursing coverage when the primary nurse is off duty
    • Maintaining a current Nursing Policy and Procedure Manual accessible to staff
    • Maintaining CPR certification
    • Maintaining a current RN license
    • Maintaining certification in Infection Control for Health Care Workers


  • Provide nursing triage for non-medically enhanced residences during off hours.
  • Screen reported symptoms, assign urgency levels, and direct appropriate action based on standardized triage protocols.
