Patient Navigator 3 F_TEP-Campus Employees Temporary Employment 88272BR Job Summary The Substance Use Disorder Patient Navigator will provide transitional support to individuals struggling with Opioid Use Disorder who are currently in custody and/or recently released from San Francisco County Jails. The Navigators will enroll and serve 200 patients over the 5 year grant period individually and in group settings with a focus on harm reduction counseling and opioid treatment linkage. Navigators will work in the jails and at the OBIC Clinic, located at 1380 Howard Street. Their ultimate goal will be to connect patients with a provider in the community who can continue their medication treatment for Opioid Use Disorder. Other objectives that Project JUNO Community will focus on will be to decrease the risk of overdose and other substance use related comorbidities, improve physical and mental health, improve employment and housing status, improve social connectedness, for patients up to 6 months after their release. Navigators will also administer standardized questionnaires at intake, 3 months, 6 months and at program discharge. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
