Job Title: Housing Stabilization Case Manager

FLSA Status: Full Time - non-exempt
Reports to: Program Manager

Schedule: M-F 40 hours
Supervises: N/A

Rate of Pay: $26.52 hourly

Benefits: Standard; Full-Time, Non-Exempt employees are eligible for but not limited to the following:

Function

The VA GPD Housing Stabilization Case Manager provides case management services to enhance housing stability and self-sufficiency in Veterans who are at risk of homelessness or who have recently transitioned out of homelessness and into permanent housing. The desire is to see Veterans increase their income and overcome barriers to housing stability. This is done through a strength-based/client-centric approach.

Duties and Responsibilities

* Act periodically as a liaison and support between external stakeholders and community professionals on case situations, ISPs, issues, and problems.

* Provide trauma informed case management services to include, budgeting, guidance/training on life skills, and landlord engagement. Engage in home visits with Veteran clients as necessary.

* Connect Veteran clients with community resources such as employment services, mental and physical health providers, and various VA service providers.

* Plan and conduct outreach efforts to identify Veterans who are at risk of homelessness in order to prevent those Veterans from falling into homelessness.

* Network with community service providers to identify potential resources for Veteran clients.

* Provide housing stabilization support for Veterans after they exit GPD transitional housing into permanent housing.

* Conduct "warm handoffs" with GPD Case Managers for Veterans who are exiting GPD transitional housing and into permanent housing.

* Monitors clients' progress and ensures that needed services are being provided, that Individualized Service Plans (ISPs) are being followed through in a timely manner and determines the appropriateness of services provided.

* Meet with Veterans assigned as needed on a case-by-case basis (weekly, biweekly, monthly) for updates and guidance.

* Prepare for and participate in training as required by the Social Services Director and VA Program Manager.

* Provide transportation to Veterans for medical appointments, meeting with landlords, VA appointments, and/or other services on a case-by-case basis.

* Serves as Mandatory Reporter.

* Complete Incident Reports; Protect the Mission (PTM) reports; and all other necessary reports that are required.

* Ensures compliance with Federal, State and County laws, outcome measures, rules, and regulations; reviews cases to ensure case plans are developed and implemented.

* Documents all contacts and ensure that all paperwork and documentation are accurately completed and filed; completes necessary case information reports, inputs updates into HMIS/Wellsky.

* Help lead and/or plan Veteran support meetings.

* Actively support the mission and goals of The Salvation Army.

* Performs other duties as required.

Education/Experience



  • Associate's degree in human services (or minimum of two years working with displaced clients), Bachelors preferred
  • Minimum of three years in Case Management or a related field

Driving



  • If the position requires driving:
  • A minimum age of 21 (for insurance provision) and possession of a valid in-state driver's license is required.
  • An MVR will be processed every year in accordance with The Salvation Army's policies.


Background Check



  • Continued employment will be contingent upon a biennial (every two years) background check that is processed in accordance with The Salvation Army's policies.
