Job Summary

Grady Health System is seeking a Nurse Navigator for Interventional Radiology (IR) and Nuclear Medicine (NM) to provide comprehensive care coordination and clinical support for patients undergoing image-guided therapies and advanced theranostic procedures. This role is ideal for an experienced registered nurse who is passionate about improving patient outcomes through coordinated, patient-centered care.

The Nurse Navigator works collaboratively with physicians, radiology teams, and interdisciplinary healthcare professionals to guide patients through complex diagnostic and therapeutic care pathways. Practicing within the professional scope of nursing and organizational clinical standards, the navigator supports patients from initial referral through treatment and follow-up, ensuring timely access to services and continuity of care.

In this role, the Nurse Navigator serves as a central point of contact for patients, families, and providers across the Interventional Radiology and Nuclear Medicine service lines. The navigator helps reduce barriers to care, supports patient education, and ensures coordination between referring providers, procedural teams, and ancillary departments to promote safe, efficient, and effective care delivery.

Key Responsibilities
  • Provide clinical guidance and care coordination for patients undergoing interventional radiology and nuclear medicine procedures

  • Educate patients and families on pre-procedure preparation, informed consent, treatment expectations, and post-procedure care

  • Coordinate diagnostic and therapeutic procedure scheduling to ensure timely access to care

  • Collaborate with physicians, radiology technologists, scheduling teams, pharmacy, and revenue cycle departments to ensure patient readiness and authorization

  • Track and monitor patient progression through specialized procedures such as Y-90, Pluvicto, uterine fibroid embolization (UFE), and biopsy procedures

  • Monitor patient adherence to care plans and coordinate follow-up and outreach for missed appointments

  • Provide ongoing patient education and clinical support throughout the care continuum

  • Support departmental initiatives by assisting with clinical documentation, quality metrics, and process improvement activities

Required Qualifications
  • Bachelor of Science in Nursing (BSN) required

  • Current licensure as a Registered Nurse in the State of Georgia

  • American Heart Association (AHA) Basic Life Support (BLS) certification

  • Minimum of three (3) years of direct nursing experience with demonstrated clinical competence

  • Strong clinical assessment, critical thinking, and communication skills

  • Ability to coordinate care across multidisciplinary healthcare teams

  • Proficiency with Electronic Medical Record (EMR) systems

Preferred Qualifications
  • Previous experience in procedural, interventional, oncology, or case management nursing

  • Certification in Radiology Nursing (CRN), Oncology Nursing (OCN), or Case Management

  • Experience working with complex or medically vulnerable patient populations
