Position Summary:

The Assisted Outpatient Treatment (AOT) Program Coordinator position will serve as the facilitator of the AOT program that targets adults with serious mental illness who have had difficulty engaging in ongoing treatment. Responsibilities include oversight of the AOT program, participation in AOT-related meetings and collaborations, coordination of staff functions to achieve AOT deliverables, court liaison functions, and provision of direct client care. This role includes advocacy, monitoring, and assistance in developing a plan to enhance the quality of life in areas of family, socialization, work/education, leisure activity and healthcare to clients being served through AOT. This role assists clients and their families with housing, community resources and benefits as well as coordinate care among multiple service providers such as the AOT Court, mental health and/or substance use treatment teams, and health care providers as needed. The AOT Coordinator is a positive team player that promotes a culture of inclusion and participation.

Responsibilities:



  • Provide daily oversight of AOT program activities
  • Assist in the development and maintenance of the Assisted Outpatient Treatment Program.
  • Develop relationships with community partners, including court personnel, community hospitals, law enforcement, family members, and other community resources as needed, communicating pertinent information as needed.
  • Attend and actively participate in Agency and community meetings as required; provide training for community partners and present at education forums as required.
  • Must be able to communicate effectively and participate in both verbal and physical de-escalation techniques. Ability to excel in a stressful, constantly changing environment is required.
  • Ensure completion of contract deliverables for AOT, including program and reporting requirements
  • Completes progress notes and other required documentation in the electronic consumer files within the required timeframe.
  • Process all intake information and forms for clients entering into AOT including preparation of petitions, treatment plans and other required clinical documents as well as compiling outcome and cost data in order to meet contractual reporting obligations; produce other required documents and materials as requested.
  • Monitor service delivery and compliance with court ordered treatment
  • Develop and coordinate a service plan involving all human service entities involved with the clients and family
  • Serve as liaison between clients, the recovery team, and the court
  • Inform supervisor, and other members of the team as indicated, of issues/concerns related to individual clients
  • Provides training and guidance to new staff in addition to current staff that need more direct assistance
  • Responsible for upholding a culture of privacy and security in a highly confidential work environment (HIPAA covered entity service area)
  • Complies with all Catholic Charities policies and procedures that involve access to and safeguarding of client Protected health Information (all forms)
  • Performs other duties as assigned

Qualifications

Job Requirements:

Physical Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision.

Non-Physical Requirements:

Education



  • Bachelor's degree in social work, criminology, sociology, psychology or related human services field required.


Experience



  • Demonstrated experience in working with individuals with severe and persistent mental illness.
  • Experience working with individuals who are diagnosed with a co-occurring disorder.


Special Skills



  • Possess and utilize effective verbal and written communication skills
  • Ability to be self-motivated, work independently and as a team member
  • Bilingual (English/Spanish) preferred but not required
  • Ability to represent the agency in a professional manner within the community


Licensure, Registration, Certification



  • Valid Washington State driver's license and minimum required liability insurance for WA State
  • Must be deemed insurable as determined by Catholic Charities' liability insurance provider
  • Agency Affiliated Counselor Registration
  • Completion of HCA sponsored Trauma Informed Care, De-escalation Techniques, and Harm Reduction training.


Employment is conditional upon:




  • Being cleared by criminal background check and fingerprinting when required



Work Schedule: Monday to Friday, 8:00am-5:00pm

Wage Range: $29.00 - $37.00 per hour, depending on experience
