Job Summary
* Responsible for coordinating and assessing all patients referred to the Access Center from theEmergency Department and Main Hospital.
* Coordinates the approval and level of care determination in cooperation with the psychiatrist,patient/family and insurance verification.
* Coordinates and facilitates outside referrals and placement if needed

• Participates in the formulation of a treatment plan,implementation and evaluation

• Implement therapeutic, evidenced based group therapy
  and psychoeducational classes to meet the needs of the patients

• Provide brief therapeutic counseling

• Assist patients with paperwork needed for FMLA and
  Short-term Disability in order to be away from work while in treatment

• Develops and maintains professional relationships with
  treatment team members, hospital and medical staff and communitypartners

Minimum Education, Experience, Training and Licensures Required

• Master's Degree in Social Work (MSW) required

• 2 years of job-related experience required

• Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred

• Active Kentucky License: LCSW, LPCC, LMFT

• Incumbent may also be an RN: Active KY RN license required. 2 years psychiatric nursing required. Must obtain ANCC certification in psychiatric/mental health nursing within one year.
