Job Title: Care Management Assistant (CMA)

Summary: Join our care management team as a Care Management Assistant (CMA) and play a central role in helping patients transition smoothly from hospital to post-acute care or home. The CMA partners with social workers, nurse case managers, patients, families and vendors to coordinate placements, transportation, and post-discharge appointments while handling related clerical tasks and documentation.

Key Responsibilities:



  • Coordinate post-acute placements (rehab, skilled nursing, other levels of care): research options, present choices to patients/families, enter referrals (Navihealth), confirm authorizations/payments and facility acceptance, complete required screenings (e.g., PASSR/Telligen), and prepare discharge packets.
  • Schedule post-discharge appointments with primary care providers and specialists: gather appointment details, align scheduling with clinical needs and patient preferences, offer warm handoffs to provider offices, and document appointment info in the EMR.
  • Arrange transportation per the transition plan: schedule pickups to facilities or home, confirm payment/authorization, and document arrangements.
  • Communicate with empathy: use caring language and patient engagement techniques when interacting with patients and families.
  • Provide administrative support: obtain and transmit medical records, faxing, data entry, and other clerical tasks as needed.


Qualifications:



  • High school diploma required; Associate's in Health Science or related field preferred.
  • Minimum 1 year hospital or long-term care experience in case management, admissions, or related role.
  • Strong customer service skills with tact, courtesy and a patient-centered approach.
  • Proficient computer, typing and data-entry skills; comfortable using phone systems and EMR applications.
  • Ability to solve basic operational problems using sound judgment and attention to detail.
  • No license required.

Salary Range: Minimum 20.52/hour - Maximum 33.88/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
