JOB SUMMARY This paragraph summarizes the general nature, level and purpose of the job. As a member of a multidisciplinary health care team, the home pharmacy clinical pharmacist provides the full range of distributive, clinical, consultative, research, and/or administrative pharmaceutical services. The clinical pharmacist selects, compounds, dispenses, drugs, medicines, other therapeutic agents; selects medical supplies and durable medical equipment if needed; disseminates drug information to members of the hospital and clinic staff, interested community groups and patients/caregivers; coordinates drug therapy and monitoring labs with providers and outside nursing agencies; Knowledgeable of the regulations and standards for sterile compounding. May participate in hospital research projects or pharmaceutical research. MINIMUM QUALIFICATIONS Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying. Education: Doctor of Pharmacy degree from a School of Pharmacy accredited by the American Council on Pharmaceutical Education. Experience: Completion of a PGY-1 Pharmacy Residency pediatric program OR PGY-1 program and 1 year of related experience OR Four (4) years of related experience License/Certification:
KNOWLEDGE These are the observable and measurable attributes and skills required to perform successfully the essential functions of the job and are generally demonstrated through qualifying experience, education or licensure/certification. * Knowledge of prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action; fate and disposition (if known), major contraindications and potential side effects.
