*Job Title: Care Management Support Coordinator* *Position Summary* The Care Management Support Coordinator provides administrative and telephonic support to care management teams. This role focuses on member outreach, inbound call handling, scheduling services, and assisting members in navigating care plans and available resources. The coordinator serves as a key point of contact for members, providers, and internal teams, ensuring high-quality service and compliance with regulatory standards.
*Key Responsibilities* * Conduct outbound calls to members to support care plan follow-ups, scheduling, and ongoing education * Handle inbound calls from members and providers, addressing inquiries, concerns, and service-related questions * Assist members in accessing health plan benefits, community resources, and support services * Provide guidance on care plan procedures, protocols, and next steps * Support member onboarding activities, including: * Sending welcome letters * Distributing educational materials * Managing correspondence * Maintain accurate, up-to-date non-clinical member records in compliance with state and regulatory requirements * Coordinate with providers and internal teams to ensure seamless service delivery * Identify and address Social Determinants of Health (SDOH) needs by connecting members to appropriate resources * Apply working knowledge of health plan programs, benefits, and available services * Perform additional duties as assigned *Required Qualifications* *Education* * High School Diploma or GED required *Experience* * 1-2 years of relevant experience required * Minimum 2+ years preferred in: * Managed care environment * Supporting individuals with disabilities * Working with vulnerable populations and/or chronic conditions *Preferred Qualifications* * LPN (Licensed Practical Nurse), LVN (Licensed Vocational Nurse), or LCSW (Licensed Clinical Social Worker) preferred *Job Type & Location* This is a Contract position based out of Farmers Branch, TX. *Pay and Benefits*The pay range for this position is $22.85 - $22.85/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
