The Assistant Clinical Manager (ACM) is a Registered Nurse leadership role that supports Home Health & Hospice Clinical Managers and Directors in delivering safe, compliant, high-quality home health and hospice care across community-based services.

The ACM provides clinical oversight during assigned coverage (including weekends/holidays and off-hours as needed), supervises and schedules designated staff, coordinates care delivery and handoffs, and serves as a point of escalation for clinical and operational concerns.

The role partners closely with interdisciplinary and administrative teams to ensure continuity of care seven days a week and adherence to organizational policies and applicable regulations.

SCHEDULE:

  • Sunday - Wednesday or Wednesday - Saturday

  • Typical Hours: 7:30 AM - 4:30 PM

EDUCATION

  • Associate or Bachelor's degree in Nursing from an accredited program.

  • Current, unrestricted RN license in the state of practice and BLS certification.

  • Hospice and Palliative Nursing certification (preferred) or willingness to obtain within 1 year

EXPERIENCE

  • Three years' experience as a Registered Nurse in Home Health and/or Hospice/Palliative care;

  • Two years progressive leadership/supervisory experience. Consideration may be given for a combination of education and experience from which comparable knowledge and skills are acquired

  • Demonstrate and apply working knowledge of hospice/home health regulations, standards of practice, and clinical workflows to promote compliance, efficiency, and high-quality outcomes; Apply clinical judgement to balance patient needs, regulatory requirements and organizational policies in care planning and delivery.

  • Maintain productivity and quality of work while managing frequent interruptions and multiple concurrent responsibilities

  • Demonstrate effective written and verbal communication skills when interacting with staff, leadership, patients, and interdisciplinary partners.
