This is a full-time benefited position working 10.25-hour shifts on various days of the week including weekend and holiday rotation. Position typically starts at 7/7:30am and ends at 5:45/6:15pm. We invite you to join our Scripps Mercy San Diego Case Management team as an Inpatient Complex Care Manager. As an Inpatient Complex Care Manager, you will be responsible for the daily management and coordination of care for patients in the hospital with Identified complex health needs, including coordination with providers, navigators and ancillary and community resources across the system and care continuum.
