We provide comprehensive skilled nursing care, long-term care, and rehabilitation services including orthopedic rehabilitation, physical therapy, and speech therapy. Specialty programs include palliative care, respite care, and wound care, allowing us to support a wide range of resident needs with compassion and clinical excellence. Position OverviewAs a Social Worker, you will support residents and their families by assessing psychosocial, emotional, and environmental needs and connecting them with appropriate services and resources. You will serve as an advocate and key member of the interdisciplinary care team, helping ensure each resident's care plan supports dignity, independence, and overall well-being. This role involves meaningful engagement with residents throughout their entire care journey-from admission through discharge-ensuring they feel supported, heard, and empowered. Key Responsibilities
