Nursing at Lakewood's Care Center

There's something meaningful about caring for people you get to know-their routines, their stories, and the small changes that tell you they need extra attention.

At Lakewood's 87-bed, Medicare-certified skilled long-term care facility, you'll build relationships with residents and families while putting your nursing knowledge into practice. Our Care Center provides care across long-term care, a specialized dementia unit, and a transitional care unit, offering team members the opportunity to work with residents at various stages of their healthcare journey. Your work will help residents maintain their abilities, receive personalized care, and feel respected in the place they call home.

At Lakewood Care Center, you'll strengthen your clinical judgment, build relationships with residents and families, and develop leadership skills while working alongside nurses, medication aides, resident assistants, and interdisciplinary partners.

We're hiring for day, evening, and overnight shifts, giving you options to explore a schedule that fits your life. Whether you prefer getting an early start, keeping your mornings open, or working overnight, we'd love to talk about where you fit on our Care Center team.

Your Role in Resident Care

As a Long Term Care RN, you'll provide hands-on nursing care and clinical leadership that help each shift run safely and smoothly. Your responsibilities include:



  • Assess resident needs, recognize changes in condition, and respond to situations requiring RN intervention.
  • Guide care decisions and update individualized care plans to reflect residents' needs and preferences.
  • Administer medications and complete treatments with careful attention to safety and accuracy.
  • Support restorative nursing programs that help residents maintain function and independence.
  • Communicate changes in condition with families and collaborate with providers and interdisciplinary partners, including during hospice and palliative care discussions.
  • Guide LPNs, medication aides, and resident assistants through clear expectations, education, feedback, and accountability.
  • Complete timely documentation and thorough shift handoffs so the next team has the information needed to continue care.
  • Participate in quality, physician, and pharmacy rounds and complete follow-up actions.
  • Follow infection prevention practices, safety standards, and care requirements.


Position Type

We are hiring full and part-time positions! We offer flexible scheduling to promote work/life balance. Available shifts include:



  • Offering flexible scheduling
  • Shifts are: 6:00AM-6:30PM; 2:00PM-10:30PM; 6:00AM-2:30PM; 5:45PM-6:15AM
  • Weekends - every third
  • Holidays - rotating


Additional compensation is available through:



  • Competitive shift differential (PM/NOC), shift pick-up, overtime

Qualifications

Qualifications



  • Graduation from an accredited nursing school.
  • Current Minnesota RN licensure.
  • BLS Healthcare Provider certification.
  • Completion of continuing education required to maintain RN licensure

You're open to feedback, comfortable asking questions, and ready to grow your clinical judgment and leadership skills. Experience in long-term care or supervising a care team is valuable, along with a commitment to helping teammates succeed and treating residents with dignity and respect.
