Overview
The SNFist Resource RN is responsible for assisting the rounding nurses and providers by triaging calls for new admissions to the SNFist Team. Serves as a point of contact person for the SNFist Team. Serve as a liaison between the Hospitalists, providers, rounding nurses and nursing homes regarding triaging patient issues in a timely and effective manner. Perform patient telephone information intake and accumulate pertinent patient data for pending admissions by the SNFist Providers. Responsible for the delivery of nursing care using time and resources efficiently integrating a standards-based framework model. As permitted by scope of practice, the RN will provide patient care recommendations as supported by physician approved protocols. Qualifications Certifications: Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA), Education: Bachelor's Degree; H.S. Diploma/GED, Work Experience: Responsibilities Telephone Interactions: As directed, completing necessary telephone communications with patients, families, pharmacies, nursing homes and/or other health care facilities. Gathering information and maintaining patient information via telephone per department specific processes and policies. Provide patient with written instructions regarding procedures, medications, or follow up instructions at the direction of the provider. Assist with forms, DME forms as assigned. Interacts with internal and external customers to facilitate appropriate nursing home admissions and continuity of care. Initiates and assists with department and external operational issues to optimize patient care. Provides leadership through communication about and validation of patient care. Works in close collaboration with SNFist Team nurses, PSR/MA and providers, nursing home DON and staff, hospitalist team, and case management to ensure patient throughput and timely admission are addressed and minimized as appropriate. Professional telephone etiquette and strong communication skills. Occasional need to travel to long-term care facilities to foster teamwork & provide education. Prepares "pre-charting" information for the SNFist providers prior to their scheduled visits including medication reconciliation and summary of pertinent medical information, as time allows. Compensation and Benefits The compensation range for this position is $33.11per hour - $53.88per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
