Description Care Manager RN in Emergency Department at Queen of the Valley Medical Center Napa, CA. This position is Part Time from 11:00am - 7:30pm Thursday, Friday & Saturdays. The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services.
