Apply Job ID: 007224 Description: Gospel Center (1554 Minford Place) $62,400 - $65,000 /year Sign-On Bonus Eligible Position Summary: This position provides facilitation using the ACS Six Stage Model of Family Team Conferencing (FTC) with families, youth, foster families, workers, service providers, and others for FTCs in a number of methods including Permanency and Goal Changes as well as other conferences as necessary. The Family Team Conferencing Facilitator completes FTC Summary forms and distributes them to all conference members and enters progress note entries in CONNECTIONS for all facilitated conferences and FTC Summary Tab information for Permanency Conferencing as required. Facilitators are part of the Permanency Program and work to assess child safety, permanency, and well-being by providing Third Party Review assessments for conferences they facilitate.
