Position Summary: This position provides facilitation using the ACS Six Stage Model of Family Team Conferencing (FTC) with families, youth, foster families, workers, service providers, and others for FTCs in a number of methods including Permanency and Goal Changes as well as other conferences as necessary. The Family Team Conferencing Facilitator completes FTC Summary forms and distributes them to all conference members and enters progress note entries in CONNECTIONS for all facilitated conferences and FTC Summary Tab information for Permanency Conferencing as required. Facilitators are part of the Permanency Program and work to assess child safety, permanency, and well-being by providing Third Party Review assessments for conferences they facilitate.

Essential Duties:



  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Facilitate six-month Permanency Family Team Conferences (FTC) and Service Plan Reviews (SPR). FTC's bring together the child's caseworker, supervisor, foster parents, parents, youth over 10 years old, and other service providers or advocates to discuss the family's permanency plan, services being provided, and progress toward permanency.
  • Facilitate other FTC conferences for Critical Decision-making including Goal Change or other conferences as determined by Children's Aid and ACS to support case progression and permanency.
  • Hold Service Plan Review conferences as necessary if an FTC cannot be held to assure that all cases are reviewed at least once every six months to assure program compliance with state and city requirements for services.
  • Complete FTC Summary forms for all conferences facilitated and enter in CONNECTIONS as a progress note as well as in the SPR tab related to the Family Assessment and Service Plan (FASP). The facilitator will distribute Summary Reports to all conference participants whether handwritten or typed.
  • Collaborate with ACS facilitators when blended conferences are required to provide third-party review of conferences facilitated by ACS.
  • Performs other duties as assigned


Minimum Qualifications:



  • Master's in Social Work (MSW)
  • 1 to 2 years' experience in foster care and adoption.
  • Bilingual (Spanish) a plus.


Key Competencies:



  • Group skills training and experience a plus.
  • Excellent writing, communication, and organizational skills (bilingual a plus).
  • Excellent interpersonal skills.
  • Flexibility and ability to work under pressure and meet deadlines.
  • Computer literate (CONNECTIONS proficiency a plus).
