Description
We are seeking a detailed-oriented Intensive Case Management Social Worker to join our team. Under the direction of the RN Assistant Director, you will manage and collaborate closely with UCLA Medicare Advantage senior patients admitted in the hospitals, SNF and in ambulatory setting, collaborate with Faculty Practice Group clinic, community resources, other providers, managed care operations, and the UCLA Health System. Your primary responsibility will be to implement, coordinate, monitor and offer options for very high risk patients with multiple comorbidities , ensuring their healthcare needs are met through effective communication and collaboration with providers, Intensive Case Management Team , and appropriate UCLA Health System units to promote quality, cost-effective outcomes. Key responsibilities include providing direct social work services, conducting assessments, implementing interventions, and making recommendations for patients and their families. You will participate in interdisciplinary activities and adhere to departmental standards related to attendance, documentation, continuous quality improvement, and compliance with departmental policies and the Code of Ethics.

Salary Range: $47.79 - $57.09 Hourly
Qualifications
  • A Master's Degree in Social Work from a school accredited by the Council on Social Work Education required. Post-masters social work experience in a healthcare setting is highly desirable.
  • Demonstrated skill in psychosocial assessments of patients/families, clinical interventions, and treatment planning with patients and families. Demonstrated work with interdisciplinary teams.
  • Excellent oral and written communication skills
  • Strong relationship building skills. Excellent problem solving ability
  • Excellent organizational and time management skills, including the ability to handle multiple conflicting priorities simultaneously. Strong ability to prioritize.
