Job Overview:

The Pharmacy Technician Medication History assists in the provision of pharmaceutical services under the direct supervision of the registered pharmacist by performing the technical, clerical and manipulative tasks involved in dispensing, preparing, and distributing all medications and pharmaceutical products for all patient age groups (includes geriatrics, adult, adolescent, pediatric and neonatal). Work and/or performance shall conform to written established hospital policies and procedures.

In addition, the Pharmacy Technician Medication History will participate in the medication reconciliation process that will include accessing the electronic medical record (EMR) in the Emergency Department (ED) and identifying patients who have been in the ED with a note in the chart indicating they will be admitted. The technician will interview the patient or their family members to obtain an accurate medication history. The technician will also need to review the currently documented external prescription history and contact the patient s retail pharmacy to resolve any medication concerns. In addition, they will need to update the patient s home medication list in the EMR. Good communications skills are required for interviewing patients and their families, as well as relaying the use of non-formulary medications to the pharmacist taking care of the patient. The pharmacist can then verify the patient s own medication or convert it to a formulary equivalent when possible. Lastly, having a working knowledge of healthcare informatics is vital for their entry and documentation of their daily activities in the pharmacy documentation database,

Qualifications:

Required:

  • High school graduate or equivalent
  • NJ Pharmacy Tech license
  • PTCB Certification

Preferred

  • 1 year of previous pharmacy experience

Scheduling Requirements:

  • Full-Time
  • Monday-Friday 2:30pm-11:30pm
  • No weekends or holidays

Essential Functions:

  • A certified pharmacy technician will participate in the medication reconciliation process that will include accessing the electronic medical record in the Emergency Department and prioritizing patients to be admitted
  • The technician will interview the patient or their family members to obtain an accurate medication history
  • The technician will update and compare the patients' home medication list in the EMR for the provider to reconcile
  • Good communication skills are required for interviewing patients and their families, as well as relaying the use of non-formulary medications to the pharmacist taking care of the patient as needed
  • Performs other duties as required by the department
